REMINDER: ALL STUDENTS IN KINDERGARTEN, 2ND, 6TH and 9TH GRADES ARE REQUIRED BY THE STATE OF ILLINOIS TO HAVE A DENTAL EXAM

Please provide policy information if you have private dental insurance or a medical card so your insurance can be billed. Remember, this will fulfill next year’s requirement for any student in Pre-K, first, and fifth grade.
